This document outlines the standard operational policies and necessary disclaimers for all fitness and nutrition services provided by [Your Name/Business Name], certified as a Personal Trainer and Nutritionist.
To ensure the efficient scheduling of all clients and respect for the coach's time, the following policies apply to all scheduled appointments (training sessions, assessments, and consultations), whether in-person or virtual:
Clients must provide at least 24 hours' notice for any cancellation or request to reschedule an appointment.
How to Cancel/Reschedule: Notification must be made directly to the Coach via text message, dedicated scheduling app, or email.
Appointments cancelled with less than 24 hours' notice will result in the forfeiture of the session. The client will be charged for the full session price or have one session deducted from their purchased package.
Note: The Coach reserves the right to waive this fee in cases of genuine emergency (e.g., sudden illness, severe weather, medical emergency). This is determined on a case-by-case basis.
If a client fails to show up for a scheduled appointment without any prior notification (a "No-Show"), the full session fee will be charged, or one session will be deducted from the client’s package.
If the Coach must cancel an appointment due to unforeseen circumstances, the Coach will notify the client immediately. The missed session will be rescheduled at a mutually convenient time, or the client will be credited for the session.
Appointments begin and end at the scheduled time. If the client is late, the session will still end at the original scheduled time, and the session will not be extended.
To encourage client consistency and adherence to fitness goals, all prepaid packages have the following expiration dates:

Extensions: Extensions may be granted for documented medical reasons, military deployment, or other serious circumstances, provided the client notifies the Coach in advance.
Risk of Injury: The Client acknowledges that participation in physical exercise, including the use of equipment, carries an inherent risk of injury. The Client voluntarily assumes all risks related to their participation in the fitness program.
Medical Clearance: The Client affirms that they have disclosed all relevant medical information and, if advised by a physician, have obtained medical clearance before commencing any physical training program.
Client Responsibility: The Client agrees to report any pain, fatigue, or discomfort immediately during training sessions. The Coach is not responsible for injuries resulting from the Client's failure to disclose medical conditions or disregard for safety instructions.
Non-Medical Advice: The Coach is a Certified Nutrition Coach, not a Registered Dietitian (RD) or Medical Doctor (MD). The services provided are for educational and motivational purposes only and are intended to provide general wellness and dietary guidance
Not a Substitute for Medical Care: The information provided is not a substitute for medical advice, diagnosis, or treatment. Clients must consult with their primary healthcare provider or a Registered Dietitian for any specific medical conditions, specialized diets (e.g., for diabetes, kidney disease, or celiac disease), or concerns regarding supplements, medications, or blood work.
No Guarantee of Results: While the Coach will provide personalized and evidence-based guidance, the Client acknowledges that the success of any fitness or nutrition program depends on their personal commitment, consistency, and metabolic factors. No specific weight loss, muscle gain, or health outcome is guaranteed.
All discussions, assessments, and records regarding the Client's health, goals, and progress will be kept strictly confidential unless required by law or necessary for referral purposes with the Client's written consent.
Data Usage: The Client agrees that anonymous, aggregated data may be used for professional development or service improvement, but individual identity will never be revealed.
By purchasing and participating in services, the Client agrees to and understands all policies and disclaimers stated above.
